Veterinary Client Referral 

If you have a client that you would like to refer for physiotherapy, please download and complete the consent form below. Once completed please email through to driscolltherapy@hotmail.com with a copy of diagnostic imaging/clinical history if necessary.

 

Once received your client will be contacted within 48 hours to discuss physiotherapy for their animal.

You will be updated with a full veterinary report once the animal you have referred has been assessed.
